It seems Twitch does now pause ads when you "tab out" of the tab, or so I've been told.
I couldn't test it on Twitch specifically, but one thing that USUALLY helps against such:
- In general, use uBlock Origin. It makes the internet bearable, and it SEEMS to works against Twitch ads most of the time.
Against this specific method:
Block the "Page Visibility API". You can either do this by adding this list to your uBlock:
https://badblock.celenity.dev/hardened/block-page-visibility.txt
Or by adding the filters yourself:
*##+js(aeld, pagehide)
*##+js(aeld, visibilitychange)

You can test it here by opening the page and tabbing or minimizing back and forth and see if the site "sees" it:

The best method: Don't use Twitch. But I know that's not a solution for everyone.
